Additional Documentation

  • For events at which alcoholic beverages are served, complete a campus agreement form and forward a copy to Erin O’Brien in advance of event.
  • For events with entertainment, security, alcoholic beverages, or other elements, you may also need to complete the online “My Events” form through Blink and route it for approvals within your unit’s hierarchy. Once completed, forward a copy to Erin O’Brien.
