Digital Audio Reserves Request Form

Please Note

The Arts Library does not accept new Digital Audio Reserve requests during the 10th or finals weeks of the quarter.

If the source recording is not currently owned by the UCSD Libraries, please write "own" in the Call Number box on the request form. Then bring the original recording to the Arts Library for processing. The UCSD Libraries’ make every attempt to purchase items requested for reserves for the permanent collection.

If the material you wish to place on Digital Audio Reserve is unpublished, the course instructor assumes sole responsibility for obtaining permission from the copyright holder(s). Usually there are separate copyrights for both authors/composers and performers. If the work was created as part of a student's course work, please have the student complete the permission form.
